The patient’s number of missing teeth and the planned fixed prosthetic rehabilitation are also factored into the evaluation.
Zygomatic implants may be evaluated in the following cases:
- Presence of severe bone loss in the upper jaw
- Long-term edentulism
- Removable dentures failing to provide adequate stability, retention, and comfort
- Insufficient residual bone volume for standard implants

When clinically indicated, zygomatic implants can be used independently (e.g., quad zygoma) or combined with standard implants placed in the anterior region (hybrid approach).
In patients with single tooth loss and adequate residual bone, zygomatic implants are generally not the first choice. Standard dental implant therapy is evaluated for these patients.
Which Factors Affect Candidacy for Zygomatic Implants?
Having bone loss in the upper jaw does not automatically mean a patient is an immediate candidate for zygomatic implant surgery. General health status, oral hygiene compliance, sinus health, and adherence to post-operative follow-up appointments are equally critical.
The following conditions may influence the treatment plan:
- Uncontrolled systemic diseases
- Active intraoral, periodontal, or maxillary sinus infections
- Use of specific medications that may compromise bone healing
- Heavy tobacco use
- Inadequate daily oral hygiene practices
- Inability to attend regular clinical follow-up visits
- Anatomical variations in the zygomatic bone or surrounding midfacial structures rendering treatment unsuitable
The presence of one of these conditions does not necessarily mean the procedure is permanently contraindicated. Certain underlying medical conditions or localized infections must simply be brought under clinical control first.
Is Zygomatic Implant Surgery Painful?
The surgical procedure is performed under profound, appropriate anesthesia (local anesthesia, sedation, or general anesthesia). The primary objective is complete pain control throughout the operation.
When formulating the anesthesia plan, the patient’s general health profile, the surgical scope of the procedure, and individual anxiety levels are evaluated collectively.
Following surgery, temporary swelling, tenderness, bruising, and mild soreness may be observed. The intensity of these symptoms varies from person to person.
Using prescribed medications strictly as directed, adhering to the post-operative nutritional plan, and attending scheduled recall appointments ensure that the recovery phase progresses smoothly and predictably.
What Are the Advantages of Zygomatic Implants?
Potential advantages of zygomatic implants include:
- Providing a fixed treatment option for patients with severe maxillary bone resorption.
- Serving as an effective alternative to extensive bone grafting procedures in select patients.
- Allowing the delivery of an immediate fixed provisional prosthesis in suitable cases.
- Reducing the overall total treatment timeline for specific patient groups.
- Facilitating the transition from unstable removable dentures to a fixed, permanent full-arch solution.
These advantages may not yield the exact same outcome in every patient. Treatment candidacy and expected clinical benefits must be evaluated on an individual basis.
